SUMMARY
Family Service League is seeking a full-time Case Manager for the Family Recovery Center in Bay Shore to provide a combination of direct clinical service, outreach, concrete assistance, reporting, and contact with collaterals. The Case Manager will be responsible for performing intake assessments for new clients, completing proper documentation, and following up with collaterals.

RESPONSIBILITIES
- Cover walk-in hours.
- The Case Manager will perform intakes on new clients.
- Perform outreach to help increase clinic census.
- Correspond and follow up with collaterals including, but not limited to, family members, hospitals, rehabs, and other providers, parole, CPS, probation, courts, Substance Abuse Assessment Monitoring (SAAM), Treatment Alternatives for Safe Communities (TASC), and other referral sources.
- The Case Manager will complete documentation and/or reports for various mandating reporting entities, including the Impaired Driver's System (IDS).
- Provide concrete assistance and coordination of services for clients, such as transportation, internal psychology appointments, etc.
- The Case Manager will provide training for new interns.
- Work closely with area school and field liaisons to ensure coordination between the school and the Family Recovery Center.
- All other duties as assigned.
QUALIFICATIONS
Bachelor's degree required. Master's degree preferred.
LMSW, LCSW, LMHC, or CASAC required.
Minimum of 2 years of experience in substance abuse treatment required.
Experience in completing psychosocial intake assessments and a good understanding of OASAS regulations and requirements in an outpatient treatment program required.
Experience working with Probation, Parole, CPS, SAAM and other various legal and clinical referral sources preferred.
Excellent interpersonal, organizational, and verbal and written communication skills required.
Ability to work as part of a multi-disciplinary team required.
Proficient computer skills, including Microsoft Office required; Electronic Health Records preferred.
Bilingual in Spanish preferred.
Valid and clean NYS Driver's License required.
PHYSICAL REQUIREMENTS
This position requires sitting and standing for long periods of time.
